IEP Needs Assessment & Intake Questionnaire
Use this guided questionnaire to organize what is happening at school, what support your child currently receives, and where you may want help. It is an intake tool for SustainBL's IEP services—not a clinical, educational, or legal evaluation.
What the questionnaire covers
- Your child's age, grade, and current IEP or special education status
- Current school supports, services, accommodations, and challenges
- Academic, social-emotional, functional, and transition goals
- Your priorities and questions for possible IEP advocacy support
Helpful information to gather first
- Your child's current IEP or 504 Plan, if one is available
- Recent evaluations, progress reports, or school communications
- A short list of the concerns and goals you most want to discuss
You can still complete the questionnaire if some records are not available. Please avoid using it for an emergency or an immediate safety concern.
What happens after you submit
Your answers give the SustainBL team context for a follow-up about your situation and possible services. Completing the form does not create an attorney-client relationship or replace an evaluation by your child's school or another qualified professional.
